Home can be the part of a treatment journey you look forward to most. Before leaving, make time for the less visible arrangements: the next review, the written instructions and the handover to your usual clinician. These details deserve a place in your plan.
Know who is responsible for follow-up, when it is due and which contact to use for an urgent concern.
Bring the information together
Before leaving, request a discharge summary and the instructions your treating team wants you to follow. Ask how your usual clinician will receive relevant records with your consent.
Confirm the handover
Write down the date and purpose of the next review and who is arranging it. Clarify how to contact the treating team after returning home, including what to do if they are unavailable.
Separate routine questions from urgent concerns
Ask your clinicians which changes require prompt medical attention. Seek local medical care if you suspect a complication; do not wait for a website message or an overseas reply.
Questions to take with you
- Do I understand the written instructions?
- Who is arranging my next review?
- How will my home clinician receive the records?
- Where should I seek help if something is urgent?
Your questions deserve a thoughtful next step.
